1. I started by cropping my chosen image into a 100x100 canvas. Try to use an image with similar original colouring, or this tut will probably go horribly wrong. ;)
2. Duplicate base, set to Screen 100%.
3. New Selective Colour layer!

YELLOWS +100 –100 +100 –50
GREENS +100 –100 –100 +100
CYANS +100 –100 –100 +100
BLUES +100 –50 –100 +100
MAGENTAS 0 0 0 –100
WHITES 0 0 0 –100
5. New Colour Balance layer. Check the 'Preserve Luminosity' box.
SHADOWS +15 –5 –25
MIDTONES +25 +7 –25
HIGHLIGHTS –25 +15 +25
6. New Brightness/Contrast layer.
BRIGHTNESS +20
CONTRAST +3
7. New layer, stamp visible, drag to top and set to Soft Light 100%.
8. New Hue/Saturation layer.
REDS 0 +35 +15
YELLOWS 0 –6 +5
GREENS 0 –100 0
CYANS 0 +35 –7
BLUES 0 +15 0
MAGENTAS 0 +7 –100
Now add your text and ... ALL DONE!
